4 UCRs A Code: Example Hauling company has a 22 tn minimum 30 tons hauled to soil treatment facility Hauling company bills 44 tons (30 tons plus 14 tons) to achieve 22 ton minimum Pre-approve hauling 30 tons using T075 & T076 Additional 14 tons listed as A-code (A001) Invoice from hauler identifying minimum charge must be submitted with claim Invoiced amount plus up to 18.5% eligible for reimbursement

11 UCRs Mark-up The difference, as a percentage, between the cost of an item or service and what the client is charged or claimed from the Fund Mark-up is limited: A code % C code - 6.0% X code % Bids % UCRs for T & M codes include mark-up

12 UCRs Laboratory & Soil Treatment Costs Base costs are diverse Incentive for continued cost savings that the program can share Savings Pass Along Invoiced amount plus one-half the difference between the UCR and the invoiced amount Lab and soil treatment costs will be reimbursed: Invoiced cost % the UCR, or Invoiced cost + ½ (UCR – Invoiced cost) UCR
