10 Claims Process Outpatient Reimbursement Claims AXA will credit your preferred bank account in the currency of your choice. *Note: All claims must be submitted within 60 days of treatment. Timeline: Upon receipt of claim form and receipts by post, AXA will process within three weeks time.

11 Reflect Geophyscial will pay 100% of employee premiums. Offshore Employees have the option to add their Spouse and/or Children* to the plan at rates subsidized 50% by Reflect Geophysical. *Children are eligible up to Age 21 Coverage for your Family

13 Adding your Family 1)Send a copy of each family members passport to: 2)You will receive a brief Health Declaration to be signed 3)You will receive a confirmation including terms of coverage and medical insurance cards for each member *For Health Declarations received by 20 February, coverage will be back-dated to 1 November 2011 and claims settled accordingly Two deadlines for addition of your dependents: 16 January February 2012
