1 Clinic in the Park Promoting Health and Wellness at the Orange County Great Park ________ ____________ _____ ______ __ A Community Collaborative Fiscal Sponsor OneOC Mission Establish an innovative model of health promotion and disease prevention in a non-traditional setting Vision Create a permanent Health and Wellness Center housed in the Great Park Clinic in the Park 2012

2 Goals Implement Clinic in the Park, an innovative model to provide access to health care, health promotion and disease prevention services Provide a venue where community health organizations, public, private and academic institutions/organizations, and individuals collaborate to provide services in a fixed location at a fixed time, (the 2 nd Sunday of the month at the Farmers Market from 10 am – 2 pm) Promote the Orange County Great Park and the Clinic together as a public venue for fostering individual, community and global health Clinic in the Park 2012

3 Assist income-eligible families to gain access to health care, insurance and related social services, and medical and dental homes Provide select health services, health education and preventive services for those in need AND phase in direct medical, dental and mental health services as well as a Nutrition Curriculum Increase access to community-based resources and the Great Parks programs Offer Health Chats with health professionals Add The Health & Wellness Field Trip and Curriculum to the Clinic and the Park Serve 2000 visitors in the first year of operation and increase capacity by 100% in 5 years Objectives Clinic in the Park 2012
