Personalisation Challenge thinking around traditional care plans – used to manage clients Create a Personal Plan – created to help clients achieve their own dreams and objectives in life
Personal Plan Co-develop the personal plan with the resident, and others Start with GOALS and ASPIRATIONS Respect their goals and aspirations Work out which skills they need to gain and link these to their goals and aspirations Make skills gains SMART
Outcomes Creates greater FOCUS – everything ties back to Goals and Aspirations which are unique & personal to the service user – like a fingerprint Creates greater DIRECTION – we know where were heading with the service user Much more POSITIVE approach – always looking forward & emphasis on gains.
