2 Subsequent to signing of the charter contract and prior to the first day of instruction, the authorized representative shall attend a full-day Exceptional Student Services training sponsored by the Arizona Department of Education. June 11, 2009 Contact Monica Moreno at if you have not received written notice of the meeting.

3 Phase I At the time of your contract signing, you will need to provide: 1.Completed I.R.S. Form W-9 1.Completed I.R.S. Form W-9 2.General Statement of Assurances form obtained from the ADE website 3.Copy of the open meeting notice sent to the Arizona Secretary of States office 3.Copy of the open meeting notice sent to the Arizona Secretary of States office 4.School site location information - Documentation of a secured facility

4 Now What?? Once your contract is signed, ASBCS will request a CTDS from ADE for your charter. Once your contract is signed, ASBCS will request a CTDS from ADE for your charter. 7 Phase II At least 30 days prior to the opening date of your school, the following documents shall be provided: 1.Certificate of Insurance issued by an insurance company licensed to do business in Arizona; 2.Evidence of a public meeting, if required by A.R.S. §15-183(C)(5), at least 30 days before the charter operator opens a site for the charter school; 3.Copy of certificate of occupancy for each school site, issued by the municipality; 4.State fire marshal/fire department inspection report for each school site; 5.County health certificate for each site at which students will be taught, if required.

8 Phase III The Board has implemented a policy that requires a charter operator to have a Certificate of Occupancy and Fire Marshals report on file in our office for each site prior to receipt of any equalization payments.

9 Changes to your Charter Contract Amendments & Notification Requests Notifications approved by ASBCS Executive Director Notifications approved by ASBCS Executive Director Amendments approved by ASBCS Board Amendments approved by ASBCS Board All amendment and notification forms must be submitted, approved and signed by both parties before the school can take action on or implement the changes requested. A copy of the governing body minutes that comply with Open Meeting Law from your board approving the amendment/notification must be provided with each request. Please mail the signed original form to our office for processing. Faxed copies are not accepted.
