Preformed confluent endothelial cell monolayers prevent early platelet deposition on vascular prostheses in baboons Peter A. Schneider, MD, Stephen R. Hanson, PhD, Todd M. Price, BA, Laurence A. Fig. 1 PTFE vascular graft preparation. Vascular grafts were covered with an outer layer of Teflon tubing and connected end to end with silicone rubber tubing. After removal of the Teflon rod, a smooth flow channel was achieved. Fig. 2 Scanning electron microscopy of graft preparations before (left) and after (right) placement in a baboon arteriovenous shunt. A and B, Untreated ePTFE graft material (30 μm internodal distance) before and after exposure to native flowing blood, respectively. C, Collagen treatment of ePTFE grafts produced meshwork coverage. D, Perfusion of collagen-covered surfaces resulted in a large amount of adherent platelet-thrombus and fibrin strands. E, Early attached ECs remained rounded and unspread, leaving areas of exposed collagen between the cells. F, After in vivo perfusion, platelets and thrombus were adherent to the exposed areas. G, Preformed, confluent EC monolayers produced complete graft coverage. H, After perfusion in the baboon, there was little platelet-thrombus evident. EC monolayers prevented significant graft-platelet interaction. The scale bar equals 10 μm. Fig. 3 Platelet deposition on various vascular graft preparations. EC monolayers were associated with minimal platelet accumulation. Platelet deposition on grafts covered with early attached ECs was similar to that on control ePTFE grafts. Grafts treated with collagen type I only were highly thrombogenic; 7 of 11 grafts became occluded and four remained for evaluation.
