The 0- h approach with very low concentrations is limited to patients with symptom onset of >3 h. Furthermore, the individual risk profile and clinical parameters (electrocardiogram, type of symptoms, patient history) must be taken into account. A patient is eligible for rule-out when the 0- h troponin measured by high-sensitivity assay (hs-Tn) is very low, or the 0- h hs-Tn is low and there is no change from 0 h to 1 h. A patient is eligible for rule-in when the 0- h hs-Tn is very high, or the change from 0 h to 1 h is high. The exact cut-off concentrations are provided in the ESC guidelines1. Westermann, D. et al.
