9 Implementation of medical pathways 9 no yes Patient in intervention group? Nurse for the elderly measures vital data according to medical prescription using telemedicine devices. Data is transmitted via smartphone to portal Receives data and makes them available to doctors and nurses. Sends SMS or Email in case of conspicuous measurements. Nurse for the elderly and doctor receive values immediately and can respond promptly. Nurse for the elderly measures vital data according to medical prescription. Data is stored in paper record. Sents summary report to doctor by email. Doctor receives regularly vital data of patient as a basis for further decisions.

10 10 Devices The Vital sensors and environmental sensors could be used successful since 2010 The video conferencing is not comfortable, not all of the problems with the audio card could be solved; we had installed two units in two apartments for the tests of the usability and handling. Since that time we have again Problems with the webcam, the sound and the quality of the video especially with the synchronization between video and audio channel Mambo is not really useful for all of the participants, because of the handling and problems with GPS receiver Results of the project

13 13 Mediplus has identified and win the following contractors for the long- term guarantee of benefits after the EU funded phase: - Housing Associations - several Health insurances - Hospitals - Medical service center for counseling and case management Furthermore, the following partnerships have been included: - German Society for Telemedicine - German Society for Health Telematics - German Nursing Associations - Technology Foundation Berlin Country (TSB) - Federal Association of Private Care Services - German Hospital Federation Plans beyond the EU funded phase

16 16 Some of the biggest health insurances has informed us that they are interested to transfer the project results into the prospective standard care system of Telemonitorig financed by the health insurances in 2013 The health insurances prefer meanwhile a methodological solution based on Propensity Score Matching and is compatible with MAST Plans beyond the EU funded phase

17 17 The German minister of health Dr. Daniel Bahr has started 2012 with a Masterplan for Telematik In 2012 the new electronic insurance card will be implemented for all of the 70 Mill. health insurance takers Plans beyond the EU funded phase
