Admission Decision Form can be found at Submit by to In order to transition in a paperless admissions process, we require the Admission Decision Form found at be sent by to Please note that Adobe Reader 8.0 or higher is required to download and enter information into this form. Once you have entered information on the Admission Decision form, you can then save it as a PDF document on your local computer and then it to If you do not have Adobe Reader 8.0 or higher, go to to download the latest version of Adobe Reader. 5 Banner Training BANNER TRAINING BANNER
Please remember that it is mandatory to attend the Banner Training Sessions given by the Registrar’s Office. Click on the link to check the available training sessions. If you have already attended Banner Training and have a Banner account log in to Banner by going to Go to IRM Banner Instances Click on Select a database based on your connection preference STES is used for testing purposes and practicing. Required of all FAU staff and faculty members All FAU staff and faculty members are required to submit the FERPA form. This form is provided to you during your first Banner Training Session. BANNER 6

The Banner Student Security Access Form is located at Select the Banner Student – Admission, Financial Aid, Registrar form and follow the directions on the form. In order to get access to Banner Admissions forms, you are required to complete the Banner Student Security Access Form which is located at Select the Banner Student – Admission, Financial Aid, Registrar form and follow the directions on the form. Use SPAIDEN to view biographic and demographic information for all persons associated with the institution Current Identification Tab Click on Current Identification Tab to view full name The first Banner Form we are going to look at is SPAIDEN. Here you will find biographic and demographic information for all persons associated with the institution. Please note that there are different tabs in SPAIDEN. Click on each tab to view the information. The first tab is the Current Identification Tab. Here you will find the student’s full name. BANNER 8

9 SPAIDEN – Alternative identification
Registrar and HR updates this information when informed of name changes Click on Alternative Identification Tab to view alternate names/IDs If you click on the Alternate Identification Tab, you will see the alternate names that the person has used. In this specific example, Human Resources had the person listed as Last Name: Student, and First Name: Test Registrar and HR updates this information when informed of name changes. BANNER

12 SPAIDEN TAB - BIOGRAPHICAL
Student must request confidential status through Registrar or Personnel/HR if also an employee HR Click on Biographical tab to view the Biographical information. If the Confidential box is checked, it means that the person has requested confidential status through Registrar or Personnel/HR Here you will also find information on the gender, birth date, citizenship, and ethnicity. 13 SPAIDEN TAB - EMAIL BANNER
Only the FAU address will be maintained Exception: Prospect’s address will be maintained when added by Admissions Click on Tab to view the addresses. The FAU address is listed under Type: FAU. This is automatically the preferred address assigned from the system. Admissions offices are entering the alternate addresses under type: PROS BANNER

14 SPAIDEN TAB – Emergency Contact
Each Emergency Contact record has it’s own entry Students are responsible for adding this data using Self Service Banner Use the vertical scroll bar to view all emergency contact entries Click on the Emergency Contact Tab to view the Emergency Contact Information. Each Address record has it’s own entry. It is important to use the vertical scroll bar to view all the entries. BANNER

15 VIEW HOLDS Use SOAHOLD to examine a student’s hold record information Use to place, remove and view hold information Many different Hold Type codes Examples: TZ The From and To date fields identify the dates that the hold is valid The Origination Code field and Description field identifies the department/office that placed the hold Note: If Release Indicator is checked, only the person who placed hold can remove it Use SOAHOLD to examine a student’s hold record information Use to place, remove and view hold information There are many different Hold Type codes Examples: TZ The From and To date fields identify the dates for which the hold is valid The Origination Code field and Description field identify the department/office that placed the hold BANNER 15

16 VIEW COMMENTS Use SPACMNT to examine a student’s comment record information SPACMNT used to place, remove and view comment information on a student’s record Anyone with update access to SPACMNT can post comments Each comment includes a Comment Type, Originator code and comment text Each comment record has it’s own entry Use the vertical scroll bar to view all comment entries SPACMNT is used to place, remove and view comment information on a student’s record Anyone with update access to SPACMNT can post comments Each comment includes a Comment Type, Originator code and comment text Each comment record has it’s own entry Graduate College will use originator GADM to display graduate admissions comments and GSTD to display graduate studies comments BANNER 16

17 SWATEST – TEST SCORE INFORMATION
Displays information about a student’s standardized test scores Best scores for GRE and GMAT at the bottom Additional information blocks – Options menu SWATEST Displays information about a student’s standardized test scores It is like screen 221 in PLUS Best scores for GRE and GMAT are calculated in the Scores and Components block For additional information blocks use the Options menu BANNER

18 SOAPCOL– Previous Colleges
Use SOAPCOL to examine a student’s previous college information SOAPCOL displays Attendance Periods and Degree Details information for each prior institution if applicable Use the vertical scroll bar to view prior college records SOAPCOL displays Attendance Periods and Degree Details information for each prior institution, if applicable. If a student has only attended FAU, SOAPCOL will be blank and Banner Self-Service can be used to view or print an unofficial transcript. If the Official Transcript box is checked, it means that the Graduate College has received the transcript and placed it on VIP. Please note that other departments such as the Registrar, Undergraduate Admissions, and Human Resources also use this form. Use the vertical scroll bar in the Prior College block to view prior college records. If a student has earned multiple degrees from the same institution, use the vertical scroll bar in the Degree Details block to view each degree. 19 SAAADMS– ADMISSIONS APPLICATIONS
Application tab : Displays application status, curricula summary and field of study summary Use the vertical scroll bar to view multiple applications The SAAADMS form contains the admissions application information. The application tab shows the entry term, the application date and number, the application status and the curricula summary. If an applicant has completed more than one application, the vertical scroll bar will be active and can be used to view all the applications. BANNER

Curricula tab Has 2 sub tabs Field of Study tab The curricula tab contains the details regarding the applicant’s proposed major, including the entry term, the applicable catalog term, the program and degree sought. BANNER

Checklist tab Displays application checklist details The Checklist tab displays all the items required to complete the admissions application record. Requirements that have been fulfilled have a date in the Received Date column. Transcripts are categorized as PT (partial transcript) or FT (final transcript) in the status field. The applicant can view the list of requirements and determine which items have been fulfilled through Banner Self-Service. BANNER

22 SAAADMS– ADMISSIONS APPLICATIONS
Comments in SAAADMS Sources, Interests, Comments Tab Displays comments placed by Graduate Admissions representatives. Users responsible for processing the Admission Decision Form will enter any conditions to admission. Graduate Admissions will copy the condition in the comment field with the Originator Code GCND These comments will be appear in the applicants’ acceptance letters to advise them of the conditions imposed by the department The Sources, Interests, Comments tab is used by the Graduate College to record conditions to admission imposed by the department. The Graduate Admissions representatives responsible for processing Admission Decision Form will enter any conditions imposed by the department in the comment field. The Originator Code GCND is used to denote a condition. The conditions are recorded by cutting and pasting directly from the Decision Award form and will appear in the applicants’ acceptance letters. Application Status Applicants can now check their application status online through Banner OWLS. Applicants are assigned a Z-number as soon the application is loaded into Banner and can search for their Z-number using the Forgot FAU Z number button. BANNER

An applicant can review each application submitted by term. Multiple applications for a single term can all be viewed by clicking the link for that term. BANNER

Application Status comes from SAAADMS, Application Tab An application summary will be displayed. The summary contains information regarding the entry term, degree, and program selected by the applicant. The Application Status and the Decision fields come directly from Banner. At the bottom of the summary, the list of requirements from the check list as well as the date each item was received is displayed. Requirements come from SAAADMS, Checklist Tab BANNER
