Semiological abnormalities (axis I: clinical characteristics) are influenced by identifiable contributions from selected pathologies, and genetic and molecular abnormalities (axis II: genetic–molecular and pathology). This classification system, inspired by the recently proposed reclassification of dystonia67, assumes that a number of recognizable PD phenotypes arise from the combination of specific clinical and pathophysiological abnormalities, which would guide the choice of subtype-specific disease-modifying treatments.
