Patient Correspondence Automatically mails to patient Working on one form that can be subscribed by all Publishes to WebCIS so staff can see if physician has mailed to patient if patient calls to inquire about results.
Narcotic prescriptions Lock up at nurses station, not to front desk! Place prescription in an envelope with patients name and Med Record # Give envelope to nurse Nurse will place in locked file on the clinic side the physician usually sees patients (i.e. Chelminski-Side 1/Colford Side 2) Check both boxes just in case Patient should be informed that they will be required to show picture ID to obtain prescriptions. Do not leave prescriptions with front desk staff.

COE/CAR Schedules will be similar to last year: SDC, Pain, DM, Coag, Preceptorship, QI QI project with presentation at the end of rotation

Current state and evolution Started Jan 2009 meeting 1 st and 3 rd Thursdays of each month Cross functional – get input and perspective from all areas on an issue 1 leader, ~10 team members MDs and CAs joined in March Areas for improvement were identified by staff and each team worked on resolutions – many have been implemented and are already improving patient care and practice efficiency
Next generation 2 co-leaders and MD sponsor Shared leadership/recorder responsibilities Coverage during vacation/absence Leadership team meetings 1/month led by Kim Y-W QI Advisors available as a resource to provide support to all teams Begin to measure progress Establish a repository for ideas and project possibilities
