2 Community Practice Residency
Advanced training opportunity with direct patient contact to enhance clinical skills, develop and implement patient care services, to become leader and innovator in community practice pharmacy and optimize transition of care. Purpose and goal. Develop our RPh for specialty therapy management.

3 PGY1 Community Practice Residencies Nationally
88 accredited community practice residency sites in US 6 in Ohio ONU, OSU, UT, Cincinnati Health Partners of Western Ohio Nationwide Children’s Hospital None in Northeast Ohio Data from ASHP

4 Program Requirements Manage and improved medication-use process
Provide evidence-based, patient-centered care and collaborate with other health care professionals to optimize patient care Exercise leadership and practice management skills Demonstrate project management skills Provide medication and practice-related information, education, and/or training Utilize medical informatics

5 Overview Meet residency requirements for PGY1 Community Pharmacy Residency Program Fulfill staffing responsibilities while completing residency Complete teaching experiences through collaborative relationship with NEOMED Provide MTM service, adherence monitoring, wellness and preventative care.

6 Community Pharmacy PGY1 Residency Program
Residency Program Director: Angie Ortenzi Advisor: Mike Wascovich Preceptors: Complete Professional Academic Record Form Complete PGY1 program over a 1-year period Longitudinal design, not monthly rotation Staffing Immunization Anticoagulation clinic Providing MTM Practice related research project Teaching at NEOMED Developing preceptor qualifications check list after academic record form complete as suggested by James Owens

7 Staffing Component 1 day per week in outpatient pharmacy
Initial training required Weekend rotation in JJ Pharmacy 1 day per week in anticoagulation clinic 1.5 day per week providing scheduled MTM service

8 Projects Drug utilization evaluation for Employee Health Plan P&T Committee One in Fall Operational Expand on current Medicare Part D MTM service, Caresource MTM, and EHP Design, recommend, monitor, evaluate patient-specific therapy for selected disease state (diabetes, asthma, hypertension) Ohio – 1st state with Medicaid reimbursed MTM service Ex: newly diagnosed diabetics from EHP, educate on diet & exercise with therapy management (wellness) Medication Use Evaluation project can be presentation at midyear
