Note multiple focal lesions in the subcortical white matter and deep gray matter on proton density (A) and T2-weighted (B) images. From these images, it is not possible to discern which lesion, if any, is acute. DW images (C, D) at the same two slice locations demonstrate a single focus of high intensity in the posterior limb of the right internal capsule (D), indicating the acute infarction. Michael B.
