including anaesthetic or blood transfusion as considered necessary by the medical authorities present. I
confirm that I have provided all necessary details to ensure that my child will be safe at the holiday
programme and understand that in the event of any injury or illness all reasonable steps will be taken to contact
me. I will inform programme staff as soon as possible of any changes in medical or other circumstances between now
and the end of the holiday programme.
