Analytics Visualization Clinical Decision Support for Patients Patient directed/authorized, sometimes provider-requested- Patient data is captured via data entry into an APP (e.g. patient portal or PCHR) or from a sensor into a remote monitor or smart phone. Patient directed/authorized, sometimes provider-requested- Via either electronic or non-electronic, or structured or un- structured means the data is transferred to the provider organization. Provider directed/authorized- Information is reviewed and documented by the provider organization in either an EHR or non-EHR system by MDs and staff.
