5 WHAT IS A FRUIT AND VEGETABLE PRESCRIPTION PROGRAM?
A nutrition incentive program where healthcare providers offer clinical care, nutrition education, and/or a fruit and vegetable prescription redeemable for fruits and vegetables at a participating retail location. Trainer Notes: Use this slide to discuss what your fruit and vegetable prescription program is. You may also want to include the benefits of the program, such as its positive impact on health and for communities by increasing consumption of and access to fresh, healthy, and locally grown fruits and vegetables; improving health outcomes in patient populations suffering from chronic diet-related diseases; or increasing the viability of local healthy food retailers by increasing the purchasing power of participants who regularly shop for fruits and vegetables. 5

19 PARTICIPANT IDS Generate a list of unique identifiers, or Participant IDs Assign IDs to each patient once they are enrolled The same ID should be used for the participants throughout the entire program Keep a master ID list that links each patient name to the ID number; store in a secure, locked file Trainer Notes: Use this slide to discuss the use of unique identifiers The Program ID number is a unique identifier that is assigned to each participant to anonymously track health metrics and prescription redemption information. It is important to note that the same ID should be used for a participant during the entire program on all forms. Keep a master ID list at your clinics linking the participant’s name to the ID, but this list should stay at the clinic in a secure electronic file or a locked file cabinet. 19

Trainer Notes: The Fruit and Vegetable Serving Size Guides are provided as an additional resource to help patients determine serving sizes when asking participants to estimate their fruit and vegetable consumption at each clinical visit. 27

28 PRE & POST-PROGRAM SURVEYS
Trainer Notes: If your program includes administering pre and post-program surveys, this slide can be used to discuss what questions are included on the surveys and at what point in the program the surveys should be administered. Reference printed surveys for clinical staff to review and ask questions during the training. 28

29 ADMINISTERING PATIENT SURVEYS
1. Distribute the survey to the patient and provide interpretation support when needed. 2. Check that the program ID is on every page of the survey. 3. Collect completed surveys and make copies of the surveys and keep the originals in a secure file on site. 4. Enter survey data into DATABASE NAME Trainer Notes: Use this slide to discuss collecting survey date and where to report survey data. Tip: Surveys should not be printed on both the front and back of sheets to make sure that participants have the opportunity respond to all survey pages. While understanding the issue of paper waste, you want to ensure a high response rate. 29

Remind clinical staff to explain to the patient: How to use the prescription Where to redeem the prescription Provide any patient handouts that include program information What happens if the patient loses the prescription (will they receive a new one or not) 31

32 WHAT IS THE PRESCRIPTION WORTH?
For example: $1 per household member x 7 days per week x 4 weeks = $28 per month Trainer Notes: Use this slide to explain how to calculate a patient’s incentive amount. An example of a variable prescription amount has been included, change as needed. This family’s prescription is worth $28 per week or $112 per month. 32

You may also want to discuss how market staff ensure that the prescription is valid It can be helpful to mention that the clinical team ensures that participants know about the farmers market, know what to expect, know the names/dates/times/locations. Some participants may have never frequented a farmers market before or have little knowledge of how to shop at a farmers market. Getting participants to feel comfortable using their prescription and shopping at the farmers market is important to getting participants to returning again and again. 37

The Redemption Log is to be used to collect the following information: Participant ID The total $ amount of tokens distributed The total $ amount spent at the market The market manager/coordinator will use the Farmers Market Online Spreadsheet to report on redemption data.
