This briefing report provides some context to the review and sets out Wood’s report recommendations and the government’s formal response

Local LSCB messages LSCB Priorities for the coming year 1.Neglect 2.Missing children 3.Developing an effective board MASH – Multi agency safeguarding hub Local SCR’s published and learning messages (Child E) Private fostering

Child E key learning Indicators of neglect not identified Lack of use of an appropriate assessment tools such as GCP Lack of co ordinated multi agency approach Unclear expectations across involved agencies, with assumptions made at differing points in the process Lack of supervision Lack of training / knowledge of neglect and how it may present

Learning contd Disguised compliance from the mother and grandmother Professional acclimatisation Lack of professional curiosity – what was it like to be a child in this house hold? Impact The review concluded that all these factors contributed to an inability to assess risk effectively and to respond accordingly. The review concluded that Child E’s death was preventable

A-Z Tracking Trends High level of out of setting Injuries – definite patterns with lack of clear follow up or expression of professional curiosity Intervention – early help not always being sought “Think the unthinkable” Public health and government guidance

Which level? Reflect on the tiers as part of safeguarding responses with an appropriately trained team member or line manager. This then supports accountability and supports with decision making and professional judgement

Levels of Need

Guide to using the Thresholds effectively Remember what the purpose of the thresholds are The lists are not exhaustive The “right help and support at the right time” Professional judgement – knowledge of a child Seek support from an appropriate service

Escalation When a practitioner disagrees with a decision or response Flow chart of process, stages and who to contact for a resolution and proposed timescales. Disagreements within and between agencies must be resolved quickly and openly How to escalate – forms

Escalation form The aim should be to resolve difficulties at practitioner level between agencies; if necessary Speak to the line manager “Up the line “ within the relevant agency. The form is used to report actions, outcome and decisions.
