2 Coordinated Entry and Assessment “ A single place or process for people to access the prevention, housing, and/or other services that they need ” Uniform screening and assessment Prioritization and program matching Connections to mainstream services More efficient access to appropriate programs for consumers: fewer calls, fewer screenings, less guesswork 2

3 Coordinated Entry Process Assess households needs. Advise households on whether or not a community resource is available to meet a particular need. Educate households on what they can expect. 3

4 Individual or family will imminently lose their primary nighttime residence within fourteen (14) days. Imminently Homeless Individual or family has a primary nighttime residence that is a public or private place not meant for human habitation or an emergency shelter or safe haven. Literally Homeless Literally homeless or at risk of becoming literally homeless due to a domestic violence situation that makes the current living situation unsafe. 15 What is it like being the first point of contact for the system? Many callers are emotionally charged. Their call to 211 is often crisis driven. Callers may lack the networks they can turn to for support(family /financial). Caller identified needs exceed available resources. 15
