All rights reserved. 3Qwest Confidential & Proprietary Release 16.0 – QORA ASR Gateway Qwest will implement the ASOG Version 41 Industry Standard changes to the ASR forms and fields decided upon at an industry level with participating members of the OBF Committees. Effective date for this release as dictated by the industry is September 20, 2010. For detailed information about the industry changes on the ASR – refer to the ATIS website: http://www.atis.org/http://www.atis.org/ Qwest will implement upfront validations in the QORA ASR Gateway that impact all ASRs being transmitted to Qwest electronically. This includes the web-based QORA (Qwest Online Request Application) GUI application, or through direct connections, such as NDM or UOM interfaces.

All rights reserved. 4Qwest Confidential & Proprietary Customer Impacts/Value How does implementation of Qwest up front editing add value for you, our customer?  The QORA GUI application is a superior alternative to paper ASR forms, offering on-line help as well as validations for accuracy.  Upfront edits streamline processing time for your request, by ensuring the data supplied by you is as accurate as possible.  Prior to the ASR being accepted into Qwest’s systems, you are notified of the errors on the ASR allowing corrections to be made, thereby reducing the need for costly re-issues of the ASR (supplements).  An accurate ASR helps ensure the accurate provisioning of your request the first time.  Accurate entries also create accurate billing, reducing the possibility for errors on your monthly bill.  Reduced cycle time due to less re-work and supplements being required.

Report Enhancement – QORA GUI  Additional selection fields of EVCI and EVCOrd have been added to the QORA GUI Reports tab.

All rights reserved. 6Qwest Confidential & Proprietary ASR (Access Service Request) Form Changes:  New Fields:  Service Request Number (SRN)  Optional field that identifies the Service Reservation Number assigned by the provider in response to a request to reserve facilities.  Existing Field Changes:  EVCI  Added Valid Entry of “B”  Valid Entry Notes:  An Entry of “B” indicates that the request is a Combination order which includes one physical port and the Ethernet Virtual Connection. An EVC form must be accompanied by a Transport or End User Special Access Form.  Added Note for Changes to the request that does not requires a cancel and reissue is from “B” to Blank.  An Entry of “A” is applicable only when the first position of the REQTYP is “S”.  CKR  Changed from “optional” to “conditional” to satisfy the EVCI=A change  QSA  Definition Note: QSA cannot be greater than “01” when the REQTYP field is “E” and the SEI field on the End User Special Access Form is populated.  Valid Entry Note: Prohibited when the first position of the REQTYP field is “S” and the SEI field on the Transport Form is populated.  ACT  Added Valid Entry Note:  “C”, “M’ and “T” are prohibited for Combination Ethernet Virtual Connection (EVCI = “B”) services.  For a stand alone request the ASR ACT represents the activity of the EVC.  For a combination request the ASR ACT represents the activity of both the physical port and the EVC.  ASC EC  Added Usage Note: Required when multiple providers are involved in providing access service and the EVCI field is not equal to “B”, otherwise prohibited.  ECCKT  Added Definition Note: The COMMON LANGUAGE Special Service Circuit Code in this field should not reflect a value that equates to the EVC circuit identification when REQTYP is “E”  QTY  Added Definition Note: QTY must be equal to one (1) when the EVCI field is populated  REQTYP  Added Valid Entry Note: “E” is prohibited for Stand Alone Ethernet Virtual Connection (EVCI = “A”) services. This field is conditional, Required when the first position of the REQTYP field on the ASR Form is “E”, “S”, "V” or “X”, the SCFA is a provider carrier system, and the NC code does not specify a virtual concatenation service, otherwise prohibited.  Diverse Circuit ID (DIVCKT)  Conditional, Prohibited when the ACT field on the ASR form is “D” or Prohibited when the DIVPON field is populated, Otherwise Optional.  Diverse Purchase Order (DIVPON)  Conditional, Prohibited when the ACT field on the ASR form is “C”, “D”, “M”, “T”, or “R” and Prohibited when the DIVCKT field is populated, Otherwise optional. NOTE: This will ensure that the new circuit requested in the ECCKT field stays diverse end to end from the existing circuit in this new DIVCKT field or stays diverse end to end from a new circuit request in this new DIVPON field. All rights reserved. 10Qwest Confidential & Proprietary EVC (Ethernet Virtual Connection) Form Changes :  New Fields:  Ethernet Virtual Connection Customer Circuit Reference (EVCCKR)  Optional field used by the customer as a cross reference to the provider EVC ID(s) and in many cases to identify the customer's end-to-end service  Associated UNI/NNI Termination (AUNT)  Identifies the UREF termination point associated with the physical port being requested on the ASR. All rights reserved. 12Qwest Confidential & Proprietary CN (Confirmation Notification) Form Changes :  New Fields:  Service Request Number (SRN)  This field identifies the Service Reservation Number assigned by the provider in response to a request to reserve facilities.  Ethernet Virtual Connection Circuit Identifier (EVCID)  This field identifies the provider assigned Ethernet Virtual Connection identifier.  When the EVCI field on the ASR Form is populated the customer is ordering an Ethernet Virtual Connection circuit.  Conditional field, required when RT field is “F” and the EVCI field on the ASR Form is populated, otherwise prohibited.  Ethernet Virtual Connection Order Number (EVCORD)  This field Identifies the provider service order number for the Ethernet Virtual Connection requested service.  Conditional field, required when the EVCI field on the ASR is populated, otherwise prohibited.  Ethernet Virtual Connection Customer Circuit Reference (EVCCKR)  This field is used by the customer as a cross reference to the provider EVC ID(s) and in many cases to identify the customer’s end-to-end service.  Conditional field, required with the EVCCKR form on the EVC is populated, otherwise prohibited.  Existing Field Changes:  EVC/VCNUM  Change title of VC NUM field to EVC/VC NUM - Ethernet Virtual Connection/Virtual Connection Number  ECCKT  Added Usage Note: Prohibited when EVCI field on the ASR Form is “A”.  ORD  Added Usage Note: Prohibited when the EVCI field on the ASR Form is “A”. All rights reserved. 13Qwest Confidential & Proprietary Interval Validation:  The system will validate that the Interval on SUP 2, 3 and 4 complies with the standard and if not the Expedite field is marked. If neither is accurate the ASR will be rejected. ARLRN Field Validation:  The system will validate that the NPA/NXX populated does not match an Access Tandem. If the entry matches an Access Tandem the ASR will be rejected. NUT/UREF Validation:  The system will validate that NUT and UREF are populated ACT=D. If not populated the ASR will be rejected. S-VLAN/S-VLAN ID Validation:  The system will validate that neither of these fields are populated. If they are the ASR will be rejected. All rights reserved. 14Qwest Confidential & Proprietary Qwest Specific QORA GUI Agent/Owner Relationship Functionality is being delivered in September that allows the capability for QORA to establish an Owner/Agent Relationship for the purposes of submitting and processing ASRs through the QORA system.  QORA will create the capability for the Owner/Agent Relationship in the GUI only.  Owner will fill out a document agreeing the Owner/Agent Relationship. Owner will sign the document and also obtain signature from the Agent prior to submitting to Qwest for activation.  This does not change any existing functionality in the GUI for authorization of submitting ASRs for customers not in an Owner/Agent relationship.

All rights reserved. 16Qwest Confidential & Proprietary FAQs Are manually transmitted ASRs impacted with this release?  Yes – the industry changes for ASOG Version 41 are applicable and the new forms must be used effective with ASRs transmitted to Qwest after 3:00 pm (MST) on Friday September 17, 2010. Do the QORA ASR Gateway upfront edits impact our company if we have our own ordering systems?  Yes, all ASRS electronically submitted to Qwest through direct connections, e.g., NDM or UOM are impacted with the new edits/validations.
