5 Materials Management Answer Documents, ID Sheets, Paper Bands Precoded Answer Documents  The preprinted information on all precoded labels and answer documents must be verified for accuracy and completeness  The information submitted on the answer document should reflect the student’s status at the time of testing  Note: Inaccurate student information may prevent the current test administration data from merging with a student’s permanent record file. 6 Materials Management Answer Documents, ID Sheets, Paper Bands Hand-Gridding Answer Documents  All answer documents and all students demographic information must be hand-gridded for students who did not receive a precoded answer document.  Test administrators must verify that the students identification information has been completed correctly before returning test materials at the end of the test administration.  All students’ information must be hand-gridded on the answer documents using a No. 2 pencil. 8 Materials Management Class ID Sheets  At least one Class ID sheet should be submitted per group (grade, subject, or course and campus  The number and placement of the class ID sheet is especially important, as it determines the way in which Confidential Student Reports (CSR) is generated and delivered.  CSR will be returned under the name (teacher, counselor, or principal), that is printed on the Class ID sheet Copyright © 2011 Pearson Education, Inc. or its affiliates. 9 Materials Management ID Sheets What is a Class and Group ID Sheet?  The Class and Group ID Sheets are used to show which answer documents fall into which category, or group, for each campus.  Campuses will not have a Campus and Group ID sheet for every group, but they should not have more than one per group. 10 Materials Management ID Sheets Void ID Sheets  Void ID Sheets are used for answer documents that will not be scored. 11 Materials Management Paper Bands What are paper bands?  Paper bands serve to keep all similar answer documents together with the appropriate ID sheet  They are distributed in the District and Campus Coordinator materials Packages.  The paper bands include the following label elements:  Campus name  Grade/Subject/Form  Package_of_ Copyright © 2011 Pearson Education, Inc. or its affiliates. 15 Materials Management Answer Document Packing List (ADPL) What is an Answer Document Packing List (ADPL)?  ADPLs are the master list showing all scorable documents in a shipment  The information on the ADPL is cross checked with the count on the Campus and Group ID sheet and the number of documents actually received.  The counts determine that all scorable materials have been returned for each campus. 16 Materials Management Answer Document Packing List (ADPL) Why is accuracy and completeness of the ADPL important?  If the quantities of answer documents returned for a campus do not match those listed on the ADPL, processing for that campus may be delayed until the discrepancy is resolved.  It is critical that all counts match-up  ADPL  Campus/Group ID Sheets  Number of documents actually received  Note: Reports for an entire district cannot be generated until it has been determined that the return of scorable materials has been complete for each campus. 18 Materials Management Answer Document Packing List (ADPL) Packing the ADPL  ADPLs are the master list showing all scorable documents in a shipment, they should be placed inside the top of Box 1 before returning to Pearson.  The materials are packed according to the order of the campuses listed on the ADPL. beginning with the last campus listed and the quantity in the far right column.
