3 Charting Temperature  Oral (O) or (PO)  Rectal (R)  Axillary (Ax)  Tympanic (AU) or (T)

4 Converting Temperature  F to C C = 1.8 / (F – 32) F = 1.8 C + 32 100 C1.8 x 100 + 32 = 212 F 101 F1.8 / (101 – 32) 1.8 / (69) = 38.3 F

5 Contraindications  No oral temp. for :  Mentally confused  Uncooperative / Combative  Unconscious  Restless  Receiving heat/cold tx about the neck/face  Mouth breathing  Difficulty breathing  Receiving oxygen by mask  On seizure precautions  Using a nasogastric tube (NG tube)  Under 6 yrs. old  Recovering from surgery/injury to the nose/mouth

6 Contraindications  No rectal for:  Diarrhea  Constipation / Fecal impaction  Had a colostomy  Combative  Rectal bleeding  Hemorrhoids  Recently had prostrate surgery  Recent heart attack  Severe heart disease
