8 Supporting Caregivers For many people, caring for older adults who are frail or have disabilities is a part of daily life. However, this kind of care giving can be very demanding and a challenge to do alone. Community Care offers Support Respite Education and Information 8

9 Who are Caregivers The CE LHIN’s April 2009 “Caregiver Support and Well-Being Priority Project” defines "caregivers" as "family members or friends who provide both short term and on going care and assistance, without pay, to those in need of support due to physical, cognitive, mental health or addiction conditions throughout the continuum of care". 9

10 Importance of Caregivers Care giving by family and friends is integral to maintaining the health and well-being of individuals living with chronic health conditions, disabilities or needs related to aging Informal caregiver roles are increasingly important, especially with the aging population Caregivers may be balancing competing demands Their stress levels may increase in parallel with the needs of the individuals they are caring for Caregivers themselves may need assistance to undertake and maintain their care giving role 10

14 Adult Day Program Activation, recreation and social supports for frail older adults, adults with developmental or physical disabilities and individuals with dementia Respite for the caregiver Six locations throughout CKL, Lindsay, Bobcaygeon, Fenelon Falls, Little Britain and Omemee 14

15 Caregiver Support Groups Connect with others who share similar experiences Explore the challenges of care giving Explore self-care Gain new knowledge and skills Share information about community resources Three groups a month; Fenelon Falls on the first Wednesday in the afternoon, Lindsay on the first Wednesday in the evening and Lindsay on the second Wednesday in the morning at Hospice Services 15

18 Community Health Centre Variety of primary care, wellness and health promotion services Stroke Survivor Support Group: Two monthly groups, one in Lindsay and one in Bobcaygeon for individuals who are recovering from strokes and their caregivers GAIN Community Team (Geriatric Assessment and Intervention Network): Goal is to keep frail seniors over the age of 65 with complex needs in their own home 18

19 Other Community Supports Home support services are also available from non profit and private businesses in CKL. Alzheimer Society – education and support groups for individuals with dementia and their caregivers Acquired Brain Injury – education, support groups and day programs for individuals with brain injury and their caregivers Personal Care and Homemaking Providers, both for profit and non-profit including VON, St. Elizabeth Health Care, Red Cross Society, Rest Assure 19
