2  Purpose: ◦ In partnership with our community and regional partners, we will offer free primary care services to the uninsured and high risk populations across dozens of central valley communities.  Community Benefits: ◦ Not everyone obtains healthcare coverage (for several reasons). Our goal is to be direct providers and bridge the gap to accessible healthcare services to promote healthier communities and provide more purposeful impact.

4  Office visits/Physicals  Wellness visits  Pap smears  Sick Visits  URI/UTI  Minor procedures: ◦ Ingrown toe nail, Wart treatment (Nitro), Minor lacerations  Low-acute Urgent Care (no ortho or large lacerations)  POC testing (urine pregnancy, urinalysis, rapid strep, finger stick glucose)  Vaccines  NO Narcotic Dispensing  Most Lab draws  Community Resource Referrals

6  Twelve outreach locations for free clinic will be supported during year one of operation.  Each clinic session can accommodate 15 patients per day or 3 patients/hour.  Collectively, we can accommodate 240 patients per year, at minimum.

7  We plan to travel across dozens of communities throughout Stanislaus, San Joaquin, and Merced counties  Rotate scheduled service areas to better serve targeted communities. ◦ Ex: Mondays can be dedicated to Merced county communities, Tuesdays for Modesto communities, etc.  Attend community event & employer events ◦ Collaborating with community partner’s unique events

9  Sharing resources and referral networking ◦ Primary care providers ◦ Pharmacy and medication resources  Opportunity to develop system-wide implementation of healthcare models ◦ Emphasis: disease prevention, chronic disease management, and ER utilization  Exemplify our foundation for regional community partnerships across our valley’s geographies and unique needs
