Practitioners would: carry out assessments, draft support plans with customers retain case responsibility carrying out reviews and re-assessments as needed assign the support plan to the brokerage team* *NB this would apply to all cases except those requiring Occupational Therapy only and a small number of complex high cost Learning Disability and Mental Health cases. Service Brokers would: develop knowledge of local services & resources facilitate innovative use of preventative services and community assets work with customers to use their personal budget to arrange support including care managed cases and self directed approaches such as DPs and ISFs ensure service use is aligned to commissioning intentions as set out in the Council’s Commissioning Strategy record framework providers’ take up of referrals finalise support plans deal with ongoing case queries and make minor support plan changes

11 Serving the people of Cumbria Do not use fonts other than Arial for your presentations Voluntary Redundancy Applications VR window open until 10 May 2016 9 VR expressions of interest received so far (to Fri 29 April) Decisions planned for VR applications (starting 24 May 2016) Applications will be considered by AD Commissioning & Integration with final approval by Corporate Director Health & Care Services

12 Serving the people of Cumbria Do not use fonts other than Arial for your presentations Assimilation Current Assimilation Policy relating to the grade match criteria challenged by GMB (not Unison) and is now subject to formal dispute process. Corporate discussions are ongoing Assimilation process will compare old and new roles to determine where direct appointments can be made Roles must be same grade and 75% match against each assimilation criteria. Where it is identified that there are more people than posts, a competitive interview process will take place Employees not assimilated to posts confirmed as ‘at risk’ of redundancy and are able to apply for vacant posts through the vacancy preference process All employees in scope will receive a personalised letter confirming the outcome of the process from w/c 13 June:  VR outcome  Confirmation of assimilation  Confirmation of VPP All letters personalised and issued via the Service Centre
