1 FDOH Children’s Medical Services - Southeast Florida Telehealth Initiative for Children with Special Needs GOAL: To develop and implement a public-private community partnership model for telehealth & telemedicine providing specialty services to CSHCN in identified medically underserved areas in SE Florida. OBJECTIVES: To provide access to quality specialty health services for CSHCN utilizing telehealth/ telemedicine technology To incorporate evaluation component relating to telehealth and health outcomes To evaluate family satisfaction and identify areas for program improvement To collaborate with universities to identify educational needs for nurses and other health professionals working with telehealth and develop curriculum materials for incorporation into educational programs

2 CMS Southeast Region Telehealth Initiative One Provider Network available to all via onsite clinics & telehealth One Region- Six Counties Maximizing provider resources (regardless of affiliation) to provide access to quality pediatric specialty care for CSHCN Daily telehealth clinics including multiple specialty sites Family choice
