4. NEEDED FOR REGISTRATION: Original Birth Certificate Social Security Card Current Immunization record (or exemption) Proof of residency(utility bill or lease agreement) Parent/Guardian Driver’s License

5. CLINIC INFO Immunization records or exemptions must be current prior to first day of school. In addition to completing the clinic packet, please be sure to let the nurse(s) know if your child has any of the following: – Life Threatening allergies (food, insect, etc.) – Asthma or other breathing issues – Specialized medical conditions – Takes any medication on a regular basis **Please be sure to send a change of clothing in your child’s backpack**

6. KINDER SCREENER INFO We will be conducting entrance screeners on incoming Kindergarteners. This will allow us to meet your child, see their current abilities and properly place them in the best Kindergarten class for him/her. 7. GETTING READY FOR KINDERGARTEN! Know on Day 1: Rules/Manners/Ready to Learn Take care of bodily functions including using the restroom independently and blowing/wiping nose independently. Fasten clothes independently, including pants and shoes. Nice to Know on Day 1: All uppercase letters, all lowercase letters, all letter sounds. How to write his or her own first name with uppercase letter at the beginning and the rest lowercase letters. How to properly hold a pencil and how to properly use scissors. Basic shapes and colors.

8. GETTING READY FOR KINDERGARTEN! Ongoing and Expected to know at the End of Kindergarten: Sight Words Rhyming Words Syllables Blending word sounds. Counting to 100 Writing sentences, short books, and poems independently. Reading books on Kindergarten Level
