2 © 2008 sanofi-aventis U.S. LLC Overall enrollment in HMOs nationwide increased after seven consecutive years of decline, to 78.3 million in 2007 from 76.3 million in 2006. The total number of HMOs operating in the U.S. fell for the ninth straight year in 2007, to 443 from 448 in 2006. Nonsystem HMOs reported 1,525.7 hospital days per 1,000 Medicare members in 2007, 7.6% higher than the average for system plans (1,417.5). HMO Industry Overview 2

10 © 2008 sanofi-aventis U.S. LLC HMO Pharmacy Overview Slightly more than half (51.2%) of all HMO plans paid for their members’ over-the-counter drugs in 2007, down from 57.1% of plans in 2006. The share of HMOs that allowed members to choose branded over generic drugs at no additional charge grew notably in 2007, to 21.4% from 12.8% in 2006. Since the launch of Medicare Part D in January 2006, the average number of prescriptions dispensed per year to HMO members who receive Medicare benefits has increased roughly 12%. 10

Between 2006 (95.8 million) and 2007 (101.8 million), the number of eligible employees covered by PPO chains increased notably. For the first time in the history of this Digest, every PPO surveyed in 2007 reported having direct contracts with pharmacy providers, up from 98.0%. PPO Industry Overview 16

20 © 2008 sanofi-aventis U.S. LLC Major Trends: HMO-PPO Digest Despite a decrease in the total number of plans (to 443 in 2007 from 448 in 2006), total HMO enrollment increased for the first time in seven years, to 78.3 million from 76.3 million. The number of government beneficiaries enrolled in HMOs climbed nearly 5% between 2006 (22,156) and 2007 (23,256). The PPO industry continued to consolidate in 2007, when a total of 572 PPOs were operating in the U.S., down from 583 in 2006, and substantially lower than the total of 988 in 2000. 20
