3 eList Tool Overview What is the eList Tool? The eList tool is a Excel Macro-enabled version of the eList Template currently used today. The eList tool has several built in security features to assist with providing the necessary information needed for Enrollment. What are the benefits of using the eList Tool my case? Similar to the eList template, the eList tool offers the same electronic alternative solution to Enrollment but with a few more benefits Multiple lines of coverage are all enrolled using one single form Clearer, cleaner submissions than written applications Output from the tool is loaded directly into our enrollment system You can easily spot missing information in the template Required information customized based on each group’s specific products/demographics Data checks help ensure we receive accurate information Many fields are simple drop down selections You use a spreadsheet to send all information instead of multiple large attachments of applications 3

4 eList Tool Overview (continued) What case size is the eList Tool used for? The eList Tool can be used for groups with 2-100 eligible lives. How does the eList Tool differ from the eList template? While the eList Tool provides many of the same benefits as the eList template, the tool has some distinct differences. Once you open the Tool, there will always be an Input and an Entry Tab. The Tool only displays the fields that are applicable to your group based on responses provided on the Input Tab. You do not need a separate list for Waiving employees. 4

5 5 Opening the eList Tool VERY IMPORTANT After opening the eList tool, it is importable that you “Enable Content” first. The ‘Enable Content’ button is found toward the top of the Tool on the yellow Security Warning strip. If the content is not enabled, the built-in macros and security features will not work If a non-enabled Tool is submitted it will cause delays in Underwriting and Enrollment.

6 How to complete the eList Tool 6 Input Tab ─Lists high level group related questions that will determine what additional information is will be asked on the Entry tab. ─Sections broken in to 6 sections Segment Effective Date Products Group Location Benefit Waiting Period COBRA/State Continuation Entry tab ─Lists EE related questions as asked on Paper Enrollment forms ─Will only display relevant fields based on responses from Input tab ─Each cell displays a pop up with required vs. optional instructions Completing the eList Tool has become more simplified. Only the Input Tab and Entry tab need to be completed. Do not attempt to alter, hide, move or delete any columns on the Entry Tab.

7 How to complete the eList Tool – Input Tab To complete the Input Tab, answer all group related questions in each section. 7

8 Completing the Input Tab (continued) Answer group questions 1.Segment ─Select the appropriate case size 2-50 or 51-100 2.Effective Date ─Select the appropriate effective date 3.Products ─Select all sold products for the group Medical and Dental have follow- up questions if answered ‘Yes’ 8

9 Completing the Input Tab (continued) Answer group questions (continued) 4.Group Location ─Enter Plan Sponsor’s HQ address as listed on the ER app. ─Select state from drop down 5.Benefit Waiting Period ─Responses should mirror answers on ER app. ─Note: If waiting period is not waived, Dates of hire will be needed for all employees. 6.COBRA/State Continuation ─Responses should mirror answer on ER app. 9

10 How to complete the eList Tool – Entry Tab To complete the Entry tab, populate the presented fields for each member (employees and dependents) based on the information provided on the paper Enrollment forms. All fields will have a pop-up with instructions that advises if the data needed is either required of optional. There are 2 sample lines (1 EE and 1 Dep) listed to provide an example of data needed for each field. Begin keying in member info on line 3. Do not remove sample lines. 10

11 Completing the Entry tab (continued) Key Notes: 1.When populating the SSN for the dependents, the EE SSN is also needed. (See Dependent Sample on line 3) 2.If a dependent lives at a different address other then the EE’s address, it should be listed in the address field on the same line as the dependent’s name. 3.If the EE works at a different work location other than the Headquarter location, the address must be provided in the “Work address, city, state & zip” fields. 11

12 Completing the Entry tab (continued) Additional Key Notes: 4.If the group is not waiving the waiting period for new hires, the Hire Date must be provided for all employees. 5.If a dual BWP is provided, the BWP selection for each employee must be indicated. 6.If the group is electing an HMO and/or DMO platform product, the primary care physician and/or dentist number must be provided. 7.If the group is in a state that is tobacco rated, the tobacco questions must be answered. 8.If an Incapacitated dependent is enrolling, the Incap forms will need to be submitted with the case file. (Forms can be provided by UW). 12 4 & 5 6 7

13 How to save the eList Tool To Save the eList tool for New Business submission: Go to File  Save As  Enter File Name  Select Save as type  Save IMPORTANT: Save the Tool with the correct file type The file extension should always be saved as: *.xlsm ─Saving the tool with any other file extension may cause delays in Underwriting and Enrollment. 13
