All rights reserved. 2013 Seasonal Flu Vaccinations Program The event schedule is set and 39 sites are holding events this year!  Prior to this call Benefits has reached out to you to set the site schedules NEW – Vendor: Affiliated Physicians NEW – No more worrying about late shipments or storing supplies!  AP nurses will bring all supplies with them to the site the day of the flu clinic, including the flu vaccine. NEW –There is no longer a need to track insurance carrier, member ID, etc. at the time of the clinic. Eligibility:  On-site flu shots are open to all First Data benefits eligible employees regardless if they are covered under a First Data Medical Plan. Contractors, interns, owner-associates scheduled less than 30 hours per week and family members are not eligible.  Employees are not eligible for a flu shot if they Are allergic chicken eggs, thimerosal, or have had a severe reaction to prior vaccines Have a fever or viral illness on the day of the clinic Have had Guillain-Barre Syndrome Are, or think they are, pregnant

All rights reserved. 2013 Seasonal Flu Vaccinations Program Online registration website  NEW - SINGLE website for all locations  Online Registration opens August 26  Owner-associates must make an appointment online by accessing the following link: https://www.apschedule.com/firstdatahttps://www.apschedule.com/firstdata  Owner-associates with out access to email may call Affiliated Physicians at (866) 481-4391 (8 a.m. to 6 p.m. ET). 7  Benefits will be sending out a communication packet to you and Corporate Communications will post a notice in Sept and Oct HR Connections as well as a site specific email 3 weeks prior to event date and reminder 3 days prior to event date.

All rights reserved. 2013 Seasonal Flu Vaccinations Program Site Scheduling and Confirmation Process  Step 1: Dates and times have been assigned for flu shot clinics  Step 2: Participate in training webinar (today)  Step 3: After this call AP will send you a confirmation email. This email will include your basic schedule (clinic date, duration, # shots requested). We need you to provide us with more info about your site, confirm the dates and return it to us.  Step 4: Three days before your scheduled clinic, we will send you a final confirmation of your clinic date/time along with the names of the RNs (via email and phone). 9

11 ©Affiliated Physicians. All rights reserved. 2013 Flow of Flu Shot events Intake & Registration Station Confirms appointment* Confirm eligibility by verifying owner-associate badge. Contractors badges are in red. Have blank consent forms available (AP staff will provide blank copies of forms) Reviews consent Staffed by First Data Site Coordinator Flu Shot Station(s) Reviews flu consent for contraindications If no contraindications, administers vaccine Employee leaves with Vaccine Information Sheet (VIS) Registered Nurse (RN) * Walk-ins – No extra flu shots will be provided at the clinic for walk-ins – If someone did not pre-register, tell them to come back at the end of the clinic and there may be leftover vaccinations due to someone who registered but was unable to attend the clinic

All rights reserved. 2013 Room set-up requirements Intake station  Can be a table either inside or outside the room but there should be space between the intake and flu station for privacy Flu station  Each working RN needs space that can accommodate a table, 2 chairs, and a wastebasket  Multiple nurse program requires multiple nurse flu stations There should be space between each station for privacy 12

All rights reserved. 2013 Affiliated Physicians Responsibilities Mid August  Coordinate and complete webinar and finalize schedules and site-specific information  Upload all confirmed clinic schedules to the registration site September/October  Monitor online registration web site  Communicate names of staff coming onsite as needed for visitor badges / site access AP team will send final confirmation email to Site Coordinators 3 days prior to the scheduled event. Confirmation email will include the names of the staff coming onsite Day of Event  Bring appointment schedule, all supplies – including VACCINE – and equipment  Complete set-up of intake and station areas; greet and check-in First Data employees  Provide employees with consent form and collect all necessary information Post-Event  Clean and pack-up, leaving room as it was before event started

All rights reserved. 2013 Site Coordinator Responsibilities Mid August  Review event confirmation, add location specific info and return the document to Affiliated Physicians by August 21.  Once you do that, your clinic will be completely scheduled! Please review information and provide the following information: Cross streets Parking Security Clearance Building Access Event Confirmations

All rights reserved. 2013 Site Coordinator Responsibilities September - October  Use marketing materials to promote events and answer questions about the program Your support is KEY to promoting this service You will be provided with a flu shot poster template. Please fill in your site information and hang posters in prominent areas in your building three weeks prior to the clinic. Keep in mind online registration ends 2 days prior to the clinic date. September & October - HRG’s & Onsite Coordinators promote and talk to managers about signing up employees, assist with registration and hang posters/distribute flyers. There are two poster versions – one is letter size and one is legal size. Please use the one that fits your facility best. Please print the flyers, cut them and place them on tables in common areas.  Monitor registration web site to be aware of appointment volume  Coordinate tables, chairs, cube panels (you can use your white screens from health screenings), wastebaskets for the event location

All rights reserved. 2013 Site Coordinator Responsibilities Business Day Before Your Event  Confirm room set-up prior to the event  Provide AP staff names to security Day of Event  Meet AP staff one hour prior to the event or earlier and escort to room  Act as a liaison for AP staff and staff the intake desk to check-in employees Any other time  Feel free to call Vanessa with any questions or if you require assistance

All rights reserved. 2013 Key Dates to Remember By August 19: AP reached out to sites to provide them with confirmations By August 21: Sites return completed confirmations By August 26: Online registration will be live (pending receipt of all confirmations) By Sept 1: First employee communication goes out! 3 Weeks before: Email from Corporate Communications will go out announcing registration 3 Days before: Email from Corporate Communications will go out announcing close of registration. October 1: Flu shot program begins!
