As they placed it on the ground one of the team had his hand between the bottom of the trolley & the ground crushing his right hand little finger resulting in the tip being amputated. As he reached over he grabbed a bar on the mixer for balance but this broke off causing him to lose his balance and fall to the ground fracturing his left ankle. The floorman who tried to hold the pup joint when the elevator released it could not hold its weight and it tipped over on to the rig floor landing on his foot. As the electrician was holding the machine’s rotary belt, the helper leant against the side of the machine and his leg accidently pressed against the “On” button. The rotary belt started to spin and pulled the electrician’s fingers into the pulley injuring two fingers on his left hand. Its weight was being held above by a fork lift and as he released the last securing bolt, the shade swung and hit the driller pushing him off the platform and he fell 3.5m to the ground fracturing his pelvis and right wrist.
