2 AISD/Children's Student Health Services 2000-20012 Case Management-Overview Goal: Implement service, implement data tracking. (Detail Follows. Report reflects initial/manual data collection.) n Services include Needs Identification, Individual Health Plan, Education, Linkage, and Barriers Identification. n Represents a new level of service quality.

3 AISD/Children's Student Health Services 2000-20013 Asthma Case Management n Number and distribution of plans is conservative. n 70% AAPs at Elementary level reflects relative need.

4 AISD/Children's Student Health Services 2000-20014 Asthma Case Management Breathing Distress Episodes and Outcomes 85.46% Students with Mod/Severe breathing difficulty remained in school. 35% of those dismissed were referred for medical care.

5 AISD/Children's Student Health Services 2000-20015 Comparison of Breathing Difficulty Outcomes: AAP vs. No AAP In Q1, students with breathing difficulty sent home more than 25% more often than other ill/injured. Q2, students with AAP sent home at same rate as other ill/injured. For SY 00-01 students lacking AAP sent home 6% more often than others.

6 AISD/Children's Student Health Services 2000-20016 Customer Satisfaction: Parent Survey-Asthma In 6 of 7 items, Parents of asthmatic students rated student health services good/excellent with >90% frequency.

7 AISD/Children's Student Health Services 2000-20017 Pregnant/Parenting Students Case Management Needs/Outcomes n Greatest Needs: Social service, insurance, child care, M.D. n Generally, there is a good level of linkage for students who identify a need. (35% needed insurance, MD) n Lower rates of insurance linkage may reflect lags in processing or information sharing.

8 AISD/Children's Student Health Services 2000-20018 RN Case Management Conclusions: n Case Management offers a focused, effective and efficient means of maintaining the health of students with chronic or significant conditions. n Case Management appears to have an impact on school attendance, and arguably, achievement. n Case Management can be used to better understand the unmet needs of students.
