Transforming Care Partnerships The new name for the Winterbourne View Joint Improvement Programme TCPs have to close all unnecessary in-patient beds by March 2019 Our TCP is in Wessex and is called SHIP In 2019 SHIP TCP will have no more than 10-15 beds per 1 million population In SHIP there are approximately 1.9 million people that means we can have no more than 19 beds Today across SHIP there are 55 patients 16 are commissioned by Hampshire CCGs (Michelle)

4. Learning Disabilities is now a Key Priority in the Five Year Forward View This means we have to do things differently – Transform Care 4

5. Two documents have been written to help Transforming Care Partnerships (TCPs) These are; ‘Building the right support’ and a new ‘service model’ so that Health and Social Care for Children, Young People and Adults work together

6. We have do things differently We have a draft plan (this means it is not finished yet) This has been based on what local people with a learning disability and their families/carers have told us

7. What we will be working on; Discharging People from hospital to local community services Early Intervention and Prevention – this means people not going into hospitals like Winterbourne View Developing Community Services – this includes; improving Hospital and GP services with ‘Learning Disability Friendly practice’s increasing annual health checks making sure services are safe and that people can do things during the day that they want to do having services in the community so as people don’t have to be in hospital Increasing the offer of ‘Personal Budgets’ for people Developing the workforce – this means making sure people have the right training and skills to support people including doctors, nurses, support workers Writing a Joint Housing Strategy

8. Aims of the Housing Strategy Identify Housing Options e.g. Living in your own home or living with others and types of accommodation; house, flat, bungalow etc Some de-registration or Residential Care Homes to Supported Living – this means changing these from care homes to people having their own tenancy and rights Provide Clear Easy Read documents to people with a learning disability and/or autism and their family that explains what options available Work with local social housing to help them to make reasonable adjustments and to make the bidding process easier Work with the Housing Support Alliance to help move people out of hospital back to the local community Health and Social Care are working with NHS Property Services to enable money from properties that are being sold, previously occupied by people with a learning disability to be recycled back to the local area
