1 Inter-professional Collaborative & Learning Environments Project: an Overview

2 Background  Western & Northern Health Human Resources Planning Forum,  Regional collaboration forum bringing together SK, MB, AB, BC, and the Territories  IPCP&LE, an example of collaboration and partnership  The project is building on the results of another initiative, the Interprofessional Education for Collaborative Patient Centred Practice (IECPCP) launched in 2005 as part of Pan Canadian Health Human Resources Plan  Western Canadian Interprofessional Health Collaborative (WCIHP), an organization derived from the post IECPCP projects in 2007 is providing expertise in research and evaluation for this project.

3 The IPCP&LE Project  Health Canada is providing $1.84M in funding to support IPCP&LE Project in Western and Northern Canada in 2010-11  The goal of this project is to develop, implement and evaluate innovative and effective interprofessional collaborative practices  Key objectives include:  defining a collaborative practice and learning environments (CL & LE)  describing the process for developing CL & LE, developing  and implementing at least one innovative model of interprofessional collaborative practice in each participating jurisdiction,

4 The IPCP&LE Project cont’  Audience: the multiple target audiences of this project include the patients, their family and communities in the project’s sties, health providers, health science students, the middle and senior managers, jurisdictional policy and decision makers  Governance: the project will be managed by a governance structure that would ensure all partners and key stakeholders have appropriate rights, responsibilities, accountabilities and common communication channels (PSC, JSC, Core Team, Site Team)  Partners: include Health Canada (funding), the Executive Director of the Forum (operational management), a contracting organization, a project steering committee, the core project team, jurisdictional steering committee, working committees, and site committees

5 The IPCP&LE Project cont’d  Provincial (Jurisdictional) Steering Committee: co-chaired by a participating RHA and a representative from the Ministry of Health (Workforce Planning Branch) is overseeing the implementation of this project in the province and coordinates cross jurisdictional aspects of the project  Project Steering Committee: a cross-jurisdictional steering committee that oversees the development and implementation of this project in all participating jurisdictions  The project is consistent with Saskatchewan’s Patient-First Review  Seen as a cross-jurisdictional opportunity to enhance patient and family- centered collaborative practice

6 The IPCP&LE Project Cont’d  Duration and Phases: The first phase of this project includes a period of 12 month  The project may be extended into the second phase for another two years subject to availability of funding

7 The IPCP&LE Project cont’  Two sites (primary healthcare teams) have been selected for implementation of this project in Saskatchewan

14 Vision of the Women’s Wellness Centre: The Women’s Wellness Centre provides a safe and respectful clinic where women are empowered to learn all they can about what contributes to their health and well-being so they can make informed choices through involvement and participation

15 Where is the Women’s Wellness Centre providing service?

16 All of the communities located within the box…  Total population 56,800 serviced  City of Yorkton (population +/- 17,600) is largest community with 31% of the total population  First Nation communities of Key, Cote and Keesekoose account for 2.8% of the population

17 Services Offered: Pre-menopause & Menopause support Sexually Transmitted Diseases clinic Birth Control Counseling Care before & after birth Pre-pregnancy planning Depression assessment Bone health support Pap & Breast exams Services are offered to women from 12 years old and up

18 Women’s Wellness Team : Physician 2 Nurse Practitioners Community Support Worker 2 Administrative Assistants Visiting Services: Mental Health Nurse/Counselor, Exercise Therapist, and are actively recruiting a Pharmacist
