Woman fills out screening form at OB/Gyn office. BP Recorded in exam room. OB/Gyn office sends forms back to Abbott Nurse tallies form and determines if referral is appropriate. Sends aggregate data to SCAI-WIN to publish findings. Individual sites get their data. Abbott sends data to third party vendor for analysis. Goals: Partner OB/Gyns with Cardiologists to raise awareness about cardiovascular disease in women Increase diagnoses and referrals of women with cardiovascular disease

9 Presentation Title Date 9Company Confidential LTXXXXXXX ©2009 Abbott Laboratories Society Partners ACC Chief Medical Officer, Dr. Solomon, described this pilot as “brilliant.” We are partnering with ACC to mine their NCDR database to identify best practices to improve PCI outcomes in women. We are partnering with WomenHeart, the largest coalition of women with heart disease, to support educational programs for women and their primary care physicians. SCAI-WIN has co-branded the screening tool and hopes to publish the results from the 3000 patients and to use these results to apply for a larger grant to expand these educational programs to improve awareness and outcomes in women with heart disease. Met with leaders from ACC, SCAI-WIN, and ACOG to discuss ways in which members of the cardiology and OGYN specialties could work together to improve screening and detection of heart disease in women.

10 Presentation Title Date 10Company Confidential LTXXXXXXX ©2009 Abbott Laboratories Expected Outcomes Each site’s multi-disciplinary team will develop an algorithm to identify risk factors and establish a pathway for patients with risk factors to be referred to appropriate specialist(s) for further evaluation. Each site will utilize a minimum number of educational “opportunities” (dinner program, lunch and learn, grand rounds, etc) to improve the awareness, screening, diagnosis, and treatment of women with or at risk for heart disease. Sites will be expected to gather metrics of number of patients screened/identified/referred/interventions over a pre-specified period. (3-6 months??) Sites will be expected to participate in minimum of 2 webcasts/conference calls – 1 to roll out program and 1 to share best practices, discuss challenges/successes. SCAI-WIN leadership will take results to NHLBI or NIH to lobby for funding for a national education program
