If items such as immunizations or other care have been missed, or are late, immediate intervention can correct the oversight before the patient leaves the institution.

18 Case Management: Utilizes informatics to assess for patient’s resources, provide insurance companies with information on severity of illness, assists patients thru the continuum of care effectively and fiscally responsibly.

20 What A Case Manager Does Performs an initial assessment including patient’s current health status and resources. Works with a multidisciplinary team to develop a safe and effective discharge plan. Works with physicians and insurance companies to facilitate hospital reimbursement for services provided.

23 Informatics has become an important tool in the delivery of health care. Without it………. PANIC!!!! Well, maybe not panic. But Informatics certainly gives the Profession of Nursing an opportunity to shine.
