2 Common problems within our local health service Long wait lists Poor communication between Specialist and GPs Inadequate information for referrals Difficulties in prioritising patients to make sure the right patient is seen at the right time Common problems faced by GPs GP seeing more patients with increasing co-morbidities Medical knowledge is constantly changing based on new evidence GPs need to have up to-date information which is easy and accessible Timely updates on local services available Common problems faced by patient’s Inconsistent patient information given Inconsistencies in services referred to Long waits to see a specialist Current State

3 ‘A Way of Working - at the primary/secondary interface’ HealthPathways - web-based health information site Assists clinicians guide patients through the complex local health system Provides support in assessing and managing patients Clear referral pathways to general community and secondary care services with up to date service directories Patient’s receiving the right care, at the right time, delivered in the right place, by the right person Jointly developed in collaboration across all sectors Is dynamic – with new pathways constantly under development and existing pathways regularly reviewed in light of changing evidence, technology, and local circumstances

5 ‘A Way of Working – building partnerships across the sectors’ Mackay HealthPathways commenced with a ‘soft launch’ in November 2014. 117 Participants across all sectors present to embark on the HealthPathways journey Pathway localisation – discussions with key Health Professionals to look at how we do business commenced December 2015 Integrated Forums – how we can improve the patients journey together Integrated medical education events showcasing completed and live pathways Mackay HealthPathways went “Live” June 2015 and now has 123 live pathways Mackay HealthPathways – a vehicle to bring services together and start breaking down the silos e.g. one joint place for education, communication of medical alerts

7 Future State - Mackay HealthPathways Create a clear, agreed and standardised working methodology jointly used by health service organisations (private, public, community) Create a single user interface that is recognised and utilised across the whole health service Create clear localised referral pathways between services Provide comprehensive service information for referring clinicians Link clinicians to the appropriate best practice information Provide support, tools and improve communication to assist GP’s in the management of patients within primary care Increase the appropriateness, quality and timeliness of referrals to specialist services Highlight opportunities for service improvement and redesign Create links between primary and secondary care clinicians
