Master ContractMaster Contract Individual Student Learning Plan Objectives Must Be: Individual Specific Measurable Attainable Timely Must address Special Education Students and English Language Learners Must provide hard copies of: Pre-Test Along with ISLP to site coordinators by 3/5/16 Mid Point Progress by 4/5/16 Post-Tests Along with End-of-year Student Report by 5/5/16

Individual Student Learning Plan (ISLP) Examples: Objectives in Mathematics 9th grade; Real Number Systems To increase students pre and post test scores from 5% to 11%; Studnets should be able to explain how the definition of the meaning of rational exponents follows from extending the properties of integer exponents to those values, allowing for a notation for radicals in terms of rational exponents. Objectives in English 6th grade; Reading Literature Students will be able to cite textual evidence to support analysis of what the text says explicitly as well as inferences drawn from the text with 70% accuracy by the end of the tutoring hours.

Liability Insurance At least $1,000,000 per occurrence Must name Alliance College Ready Public Schools as additional certificate holder Must notify Alliance Home Office of any material changes to Certificate of Insurance Master ContractMaster Contract

Please Read the Vendor Packet Thoroughly Clearance Requirements Finger Prints In accordance with Education Code Section § 9 45125.1, Providers shall fill out Life Scan application and return it with copy of Driver’s License and receipt. Medical/Tuberculosis Clearance for all Provider’s employees, volunteers, and contractors is needed to provide services to any of the Alliance College Ready Public School students

Master ContractMaster Contract Child Abuse and Missing Children Reporting Provider assures the Alliance College Ready Public School that all staff members, including volunteers, and/or sub contractors are familiar with and agree to adhere to child abuse and/or missing children reporting obligations and procedures under California law, including but not limited to, California Education Code §49370 and California Penal Code §11166 et seq. Provider agrees to provide annual training to all its employees regarding mandated reporting of child abuse and missing children Provider agrees that all staff members and/or sub contractors will abide by such laws in a timely manner.

Use of School Facilities During Services Review Alliance College Ready Public Schools Facility Usage Policy Alliance College Ready Public Schools Application for Use of School Facilities Form Submit to site. Assistant Principal will review facility availability and submit to Principal Office for approval/ denial. Due January 15, 2016 (if applicable) Providers will be notified of facility availability

Attendance ProceduresAttendance Procedures Providers maintain daily records of student service provided, including: Name/address of student Name of Provider's employee who rendered the service Dates and amount of time of such services Provider send a copy of attendance records to Ouchi-O’Donovan 6-12 Complex at the end of each month: March 5 th April 5 th May 5 th

Invoices Providers submit to Ouchi-O’Donovan 6-12 Complex two invoices: Mid-point Student Progress Report April 5, 2016 End-of-Year Student Report May 5, 2016 Itemized by name/address of student, service provided and actual number of hours for which services were provided, and amount owed Ouchi-O’Donovan 6-12 Complex will process payments to Provider within sixty (60) days of submission of such invoices, except in those situations identified in the following slide

Right to WithholdRight to Withhold Ouchi-O’Donovan 6-12 Complex may withhold payment to Provider, on ten (10) working days written notice of such withholding, when in the opinion of the Ouchi-O’Donovan 6-12 Complex: Provider’s performance, in whole or in part, either has not been carried out or is insufficiently documented. Provider has neglected, failed, or refused to furnish information or to cooperate with the inspection, review, or audit of its program, work, or records. Provider has failed to submit the invoice in a timely manner. Provider does not provide a TIN to Ouchi-O’Donovan 6-12 Complex IAW IRS Regulations

SES RecruitmentSES Recruitment Please do not pose as Ouchi-O’Donovan 6-12 Complex Personnel Please do not promote services on school property (other than Provider Fair) Please do not misinform parents that their child automatically receives SES services Please do not fill out or submit applications on behalf of parents

Computer and Internet Service Computers Must be on the SBE SES Application and approved by CDE Cannot be used as a marketing technique promising that students will receive “free” electronics Internet Service Parents may not be asked to pay upfront for internet service to be connected
