It is there to improve our health and well being, supporting us to keep mentally and physically well, to get better when we are ill and, when we cannot fully recover, to stay as well as we canto the end of our lives. It works at the limits of science – bringing the highest levels of human knowledge and skill to save lives and improve health. Doctors, Nurses and other Health Professionals will be making the decisions We want you to have more care closer to your home All of our practices will be involved There will be innovative projects across the whole of Sheffield We will ask patients and the public for input into every decision We will achieve the highest standards for all patients We will manage change well for the benefit of our patients The patient truly is at the heart of our decision – more important than organisations - our commissioning is based on what patients need and GPs are best placed to understand that We will work with many professional colleagues to ensure we have a comprehensive approach to commissioning services that understands the contribution our MDTs make to patient care. (CRG is worth flagging up at this point) Fundamentally we are about ensuring high quality services with as much that can be provided out of hospitals as is both practicable and clinically desirable, this will mean re-shaping hospital services to better reflect what patients need As a large CCG with strong academic interests, we will work with partners to bring research and innovation into our commissioning. We have contributed to the development of the Academic Health Science Network for Yorkshire and the Humber, the National Centre for Exercise and Sports Medicine and our practices are actively involved in research studies (we are the 7th highest recruiter of patients across primary care organisations in England) We are committed to practice engagement through our strong localities
