The State wants to share information with you about Supporting Individual Success and also hear what you have to say.

6 What the State is trying to do & why
The State wants: You to get the services & supports you need to live in the community. This also includes people waiting for services!

7 Services You Get The state wants to make sure that the services and money people get are: Fair for everybody Based on your needs and choices Available when you need them

12 What is the Supports Intensity Scale?
The SIS measures a person’s support needs. This is done during an interview.

What support do you need? by asking you and other people who know you well a set of questions. 13

16 About your Actual Budget
Your Budget for services is made up of 2 parts: A budget is how much money you have and how you plan to spend the money. Where you live What you do during the day. Other services like therapies, career planning, employment specialist services, and assistive technology are added on to your base budget if you need them.

18 Review What’s the project name? What’s it do?
If you could ask any question, what would you ask the person in charge?

19 What Do I Need to Do? Think about what a good life means for you.
Think about what you enjoy doing. Think about the possibility of getting paid to do something you enjoy. Get out and give back. There are many ways to be a part of your community.

20 What Do I Need to Do? Speak up. If something doesn’t seem right about your supports, tell someone. Make choices that are right for you. Think about the life you want to live and choose the supports you need. Having freedom and flexibility comes with responsibility. Make sure you are prepared. Make sure that your service plan gets you the supports you need.

21 What else is important for me to know right now?
No changes have been made to your services at this time. Information about levels is for review only. Based on review and public comments, changes may be made to what has been proposed. Change process will begin next year, starting in July Information is posted on the OADS website: OADS wants YOUR feedback & comments!
